Before your First Appointment:
We are required under federal regulations called HIPAA to give you notice at the beginning of treatment of our policies for handling confidential information. We feel it is important for you to understand these policies but we know the documents may be long and detailed. We also would like you to understand the therapy relationship from the outset. We hope that by providing the following forms online, it will facilitate your ability to review the documents and bring completed copies to your first session.
If you are coming to Wake Family Psychology, PLLC to address a Child or Teen Problem, go to the Child & Family subpage of Getting Started. You will need to complete the Client Data Form and sign the next to last page of the Treatment Agreement Form. Bring these documents along with your insurance card and any other information you think may be helpful (e.g., report cards, previous psychological testing reports, list of medications, etc.) to your first appointment. We can make copies if you do not have access to a copier.
Couples or adults should go to the Adult & Couple subpage of Getting Started to find appropriate forms that should be reviewed and filled out before the first session. Bring the Data Form and the signed next-to-last page of the Treatment Agreement Form to your first session.
